Accident & Incident Investigations
This can be undertaken by our consultants to discover the root and underlying causes behind an accident or incident and thereby prevent further recurrence. On conclusion of the investigation, a comprehensive report will be issued to the client.
- Accidents are incidents which result in injury to someone or damage to property.
- Incidents are undesired events that could have caused damage, death, injury or ill-health. Incidents can be categorised into near misses or accidents.
- Near misses describe incidents where no loss or damage to persons or property has occurred but usually only through luck.
- Dangerous occurrences describe incidents where damage has occurred such as the unintended release of a load but no injury to persons.
Accident Analysis Techniques
These will identify the hazards and look at how organisational influences, unsafe supervision, pre-conditions for unsafe acts and the unsafe acts themselves have, through failed or absent defences, contributed to loss.
Investigations
We will look at all necessary evidence including: taking witness statements; examining tools and equipment; verifying if employees have the necessary skills, knowledge and experience; whether risk assessments and safe systems of work are suitable and sufficient; whether correct PPE was being worn; and the health and safety culture within the organisation.
RIDDOR 2013
This requires certain types of specified injuries, industrial diseases, dangerous occurrences, and those resulting in an employee being absent from work for more than 7 days to be notified to the Health & Safety Executive. Our consultants can, where required, undertake this duty on behalf of the employer.
